Each 'episode' (1-3 sessions) has a definite narrative arc, which may or may not be connected, but the season (campaign) has an overarching narrative arc that has a definite plot, even if the players don't know it. I run a sandbox style, and 99% of the time I let things develop as they do, as I am very much a make-it-up-as-I-go-along GM, but at the same time I keep that overarching story in the back of my head.
